Lower Blepharoplasty Overview

Treats: Sagging in the lower eyelid, bags, and some wrinkles

Side Effects: Patients may experience swelling or bruising. Temporary double vision and blindness may also occur, but should remedy themselves within a day. Eye itching, dryness, and other discomfort can be medicated with eye drops or other prescriptions.

Risks: Anytime a patient has any kind of surgery, there is a risk of bleeding, infection, scarring, or side effects from anesthesia.

Recovery: Recovery from eyelid surgery generally takes several weeks. Within two days to a week, the stitches will be removed. In the first week, patients will want to make sure that their eyes receive plenty of rest. The redness and swelling occurring after the blepharoplasty procedure will fade with time.

Lower blepharoplasty is an eyelid surgery that creates a smoother transition from the lower eyelid to the cheek. There are many technical variations Dr. Kulbersh might use during a lower blepharoplasty to customize the eyelid procedure. The following are the steps taken for each concern:

  • The loose or wrinkled skin of the lower eyelid – A variety of modalities can address the aging skin. The excess skin can be removed by making a small incision underneath the eyelashes. If the skin has begun to thin, a chemical peel or laser resurfacing treatment can stimulate collagen production and rejuvenate the epidermis.
  • Excess puffiness or bags under the eyes – Excess fat that herniates from behind the eye may lead to puffiness in the lower lid. Dr. Kulbersh repositions the fat to the cheek area during the procedure. A small incision will be made either under the eyelashes or inside the lower lid so that the scar is well hidden. There are instances where the fat will need to be sculpted or removed. Since the face will lose volume over time, Dr. Kulbersh will not use aggressive fat removal of the lower lids. This common practice in previous eyelid surgeries results in a hollow appearance years later. Dr. Kulbersh has revamped his technique to create a more long-lasting result.
  • Loose or sagging eyelid – If the eyelid is drooping, it may need tightening with a canthopexy procedure. A canthopexy creates a firmer, smoother contour to the lower eyelid.
