The loose or wrinkled skin of the lower eyelid
A variety of modalities can address the aging skin. The excess skin can be removed by making a small incision underneath the eyelashes. If the skin has begun to thin, a chemical peel or laser resurfacing treatment can stimulate collagen production and rejuvenate the epidermis.
Excess puffiness or bags under the eyes
Excess fat that herniates from behind the eye may lead to puffiness in the lower lid. Dr. Kulbersh repositions the fat to the cheek area during the procedure. A small incision will be made either under the eyelashes or inside the lower lid so that the scar is well hidden. There are instances where the fat will need to be sculpted or removed. Since the face will lose volume over time, Dr. Kulbersh will not use aggressive fat removal of the lower lids. This common practice in previous eyelid surgeries results in a hollow appearance years later. Dr. Kulbersh has revamped his technique to create a more long-lasting result.
Loose or sagging eyelid
If the eyelid is drooping, it may need tightening with a canthopexy procedure. A canthopexy creates a firmer, smoother contour to the lower eyelid.
Loss of Volume in the Cheek
If a deflated cheek contributes to the circles underneath the eye, Dr. Kulbersh can use facial fat transfer, facial fillers, or a cheek implant to restore volume to the cheek area.

A lower blepharoplasty will last forever. The under-eye area will always look better than if a patient never had surgery. With that said, all patients will continue to age and see changes in the area under the eyes. Generally speaking, a lower blepharoplasty will last 10-20 years.
